By completing and submitting this electronic form, I (“Client”) acknowledge and agree to the following Waiver and Release of Liability.

Scope of Activities

This waiver applies to all training, coaching, instruction, and related activities provided by Christine Van Diest, whether conducted at Christine’s gym, my home gym, online, at any other fitness facility, or during any retreat, including but not limited to retreats in the United States and on the Camino de Santiago in Spain and Portugal. This waiver remains in effect for the entire duration that I am a client of and work with Christine Van Diest.

Assumption of Risk

I understand that physical exercise, training, and participation in retreats and travel-related fitness activities can be strenuous and may involve inherent risks, including the risk of serious injury, illness, or death. I am advised to obtain a physical examination from a licensed physician before participating in any exercise or training program.

I acknowledge and agree that I engage in all physical activities entirely at my own risk. I voluntarily assume full responsibility for any risks, injuries, illnesses, or damages that may occur as a result of my participation.

Nutrition and Dietary Guidance

I understand that any recommendations regarding diet, nutrition, food supplements, or weight reduction are provided for educational purposes only. I accept full responsibility for any dietary changes and agree to consult with a qualified physician or healthcare professional before making any changes to my diet or supplement use.

Release of Liability

This waiver and release of liability includes, without limitation, any injuries, losses, or damages that may occur as a result of:

• (a) My participation in any training session, activity, or program; and

• (b) Instruction, training, supervision, coaching, or dietary recommendations provided by my Personal Trainer, Christine Van Diest.

I acknowledge that I have carefully read and fully understand this Waiver and Release of Liability and that it constitutes a complete release of liability. I expressly agree to release and discharge Christine Van Diest from any and all claims, demands, or causes of action for personal injury, illness, death, or property damage arising from my participation, whether known or unknown.

Negligence

To the fullest extent permitted by law, this release includes claims based on negligence on the part of Christine Van Diest, except where such release is prohibited by statute or applicable case law.

Severability

If any provision of this Waiver and Release is found to be invalid or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect, and the invalid provision shall be severed.

Electronic Acknowledgment

By submitting this form electronically, I acknowledge that I understand and agree to all terms contained herein. I understand that this Waiver and Release may not be modified orally and that electronic submission constitutes my legal signature.

Minors

If the Client is under the age of 18, this form must be completed and signed electronically by a parent or legal guardian.
